Licensing - Small Lottery Application

You can upload up to 25 files. You cannot upload video files; supported file types are bmp, csv, doc, docx, xls, xlsx, pdf, mail, email, gif, jpg, jpeg, png, html, rar, rtf, txt, tiff, xml, zip, msg, dwg, ppt, pptx.

You will not be able to submit your evidence online if:

• you try to upload more than 25 files
• the total size of all the files you upload is more than 35mb

We check your files for viruses before we accept them. It may take a few minutes to scan your files. Do not refresh the page before the scanning has finished, as you may lose the information and have to start again.

Please make sure the whole of the document can be seen and the image is clear. In some cases, we may still ask to see the original document(s).

You need to click the Browse button to find and select your files. You can then pick a category for each one (if available) and click the Upload button.

The application will be refused if in the period of five years ending with the date of the application:

(a) an operating licence held by the society has been revoked under section 119(1) of the Gambling Act 2005, or
(b) an application for an operating licence made by the society has been refused.

The application may be refused if the local authority think that:

(a) the society is not a non-commercial society,
(b) a person who will or may be connected with the promotion of the lottery has been convicted of a relevant offence, or
(c) information provided in or with the application is false or misleading.

I make this application on behalf of the society referred to in “Society details” and have authority to act on behalf of that society.

I will make the payment of the registration fee.

I confirm that, to the best of my knowledge, the information contained in this application is true. I understand that it is an offence under section 342 of the Gambling Act 2005 to give information which is false or misleading in, or in relation to, this application.
